Psychological Assessments

Dr. Kotrba offers psychological assessments for:
-  Autism Spectrum Disorders (Autism, Pervasive Developmental Disorder, and Aspergers Disorder)
-  Attention Deficit/Hyperactivity Disorder
-  Learning disorders, including (but not limited to) Dyslexia, Disorder of Written Expression,
Mathematics Disorder, Reading Disorder, and Nonverbal Learning Disorder

Psychological assessments include the following components:

Initial Clinical Interview:  This session consists of obtaining background information, including a detailed educational, medical, and psychological history.  Present concerns will be examined in depth to determine if testing is indicated and, if so, Dr. Kotrba will explain the appropriate tests to be administered.

Psychological Testing with Child: Psychological testing can consist of intelligence testing, educational testing, personality testing, adaptive behavioral assessment, and social communicative ability assessment.  This process generally lasts 3-5 hours, and is separated into two to three different testing sessions.  Furthermore, Dr. Kotrba will consider any past assessments, school performance (utilizing past report cards), and obtain (with your approval) teacher ratings and comments concerning academic performance and behavior.

Parent Conference:  This final conference will consist of reviewing and discussing test results and recommendations.  Parents will leave this session with a "game plan" on how to proceed in order to best help their child.

Fees:
            Initial Clinical Interview: $225*

Psychological Assessment and Parent Conference: $2175-2625 (costs vary by individual need)

           * The initial clinical interview fee will be waived if a complete psychological assessment is completed.

Dr. Kotrba is out-of-network with insurance companies.   Many insurance plans cover all or part of the costs of psychological services. If you plan to file for reimbursement of treatment with your insurance company, receipt are given for personal submission to your insurance company so that they can reimburse you directly. Payment is expected at the time service is rendered (by cash or check).
