Please download, print, and fill out applicable forms prior to the first visit.

1.  All ages should view the HIPAA information form and sign the HIPAA signature form. 
2.  Depending on age, please fill out a Developmental History form for the patient. 
3. Complete a consent form (either for individual treatment or psychological evaluation). 

4.  All ages should view and consider the media release form. 

All four forms should be brought to the intake visit.
